We look forward to meeting you for your new patient appointment. You will need to arrive at least 20 minutes prior to your appointment time to complete the registration process and be seen by the doctor on time. Please bring your insurance card(s) and a photo ID to your first appointment. Please select a form from New Patient Pack Online to complete the paperwork in advance and bring it with you to your appointment.
Additionally, it would be extremely helpful if you bring the following:
- Your current medication list.
- Your medical records.
- The most pertinent information from your records (including medication list, reports of procedures/surgeries performed, immunizations/vaccinations, EKG, recent office notes, and lab work).
If your insurance company is an HMO, you will need to contact your carrier prior to your appointment to assign our doctor as your primary care physician. We will need to verify that you have selected our doctor as your primary caregiver before you are seen. Otherwise, your insurance may not cover your visit.
You are still responsible for any co-insurance, co-payment, or deductible that may be applicable.
Please be prepared to pay your co-pay at the time of your visit. We accept cash and personal checks.
In the event you are unable to keep your appointment, we will require 24 hours advance notice; otherwise, we will charge you a missed appointment fee. You may call our office at 561-495-1515 to reschedule between 9:00 am – 5:00 pm on Monday thru Friday.
Our office is closed for lunch between 12:30 pm – 1:00 pm each day. The answering service is available after hours to accept emergency calls. Call the office during regular business hours for any routine issues. Your provider may order blood tests or other studies to be completed before your next visit. If anything is abnormal, your provider will call you immediately to discuss the results.
Prescription Services
Please notify the office 2-4 days before your refill is needed. It will take 24 to 48 hours to process your request (excluding weekends).
Prescription medications are an important aspect of any patient’s care. We ask that you carefully follow all instructions, question anything that you don’t understand, and bring either your medications or a list of your current medications to every office visit so that you can review them with your provider.
Refill Policy: New prescriptions and medication changes must be taken care of during your appointment.
- For prescription refills, please call your pharmacy directly.
- Please call your pharmacy to confirm that your refill is ready for pick-up.
- Refill requests are accepted Monday through Friday from 9:00 am – 5:00 pm.
- The pharmacy will contact our office if additional information is needed.
- The nurses will contact you only if there is a problem with your request.
Referral Service
For a referral to the specialty office, please call our office at 561-495-1515, and leave the following information:
- Your name
- Date of birth
- Your phone number
- The name of your Insurance company
- The name of the specialist and when and why you will be seeing them
Please check with your insurance company to make sure your specialist is covered under your plan. Please note we do not make confirmation calls or send copies of the referral to anyone other than the specialist. Referrals take 3 to 5 business days to process. Please be aware that some out-of-network or out-of-state referrals can take up to 30 days to process.
Accessing Medical Records
Release of Records to a Specialist: If you need to have your medical record sent to a specialist for an upcoming appointment, please call our office at 561-495-1515 and have your name, your date of birth, the specialist’s name, the date of your appointment, and the phone or fax number of the specialist.
For a Complete Copy of Your Medical Record: If you need a copy of your medical record for a transfer to a new PCP, or other insurance requests, you must sign the Information Authorization Release and fax or mail it to our office. Please allow up to two weeks for your request to be processed. There is an administrative fee for copying your medical record, depending upon the number of pages copied. We must receive payment prior to sending out the copy. If applicable, you will receive an invoice, by mail, based on your request.
